New Student? Get the Information Needed for Enrollment
  

                                      
If you are coming from another school district you will need to bring the student's:

  • Birth Certificate
  • Social Security Card
  • Immunization Record on Tennessee Form
  • Recent Physical Examination
  • Achievement Test Scores
  • Current Report Card
  • Proof of Address

To expediate the time needed to complete the enrollment process, print and complete the forms to the left and bring them with you. These forms are also available in the Guidance Office.



If you are coming from another Metro-Nashville Public School you will need to bring the student's:

  • Transfer/Withdrawal Papers
  • Current Grades
  • Proof of Address
